Equipment
issued to each player:
(items to be returned at the end of the season)

Pants, game Jersey
and socks (players keep game jerseys and socks)

Helmet
with face mask and chin strap

Shoulder, thigh & knee pads
Each player must
provide their own:

All
foot wear including shoes & practice socks (must not have detachable/screw in
cleats)

Athletic cup/supporter

Mouth
Guard (Colored), must attach to helmet (custom mouth guards made by dentists are
optional)
In addition each
player must provide:

Completed registration / Consent form

Proof
of Age (copy of birth certificate - don't send original, it will not be
returned)

Proof
of Satisfactory Standing in School (copy of last report card or letter from
school official)
Equipment deposit &
Player Fee's
Equipment deposit $125.00 (separate check from players fee, please make check
payable to the league). The deposit will be retained (will not be cashed or
deposited) and returned at conclusion of season upon return of league owned
equipment in proper condition.
Non-refundable player's fee $105.00 (check,
cashiers check & money orders accepted; Made payable to: Duck Creek Pop
Warner). Fee includes Jersey which players will get to keep, and rental of
all other required equipment.

Safety is a
priority in the Pop Warner football program!
Each player must have a signed statement of medical clearance from a
examining physician that the candidate is physically fit and there no
observable conditions, which contra-indicate him/her playing football.
Note: If regular school medical examination was preformed after the
first of the year (for the season you are signing up for), and the results
are releasable to parents, a copy of such report may be used. If a physician
feels that a physical done in the prior year is still valid, then a note
signed in this year is needed.
The league policy towards unsportsmanlike like
activity (i.e. fighting, intimidation, kicking, bad language or disrespect
for authority etc. as game officials determine) is as follows:
If flagged for an
infraction of this type, there will be a 15 yd. penalty and immediate
disqualification from that game.
Any player ejected from any game for cause shall be ineligible for
participation in their next game.
A second ejection during the same season shall be cause for removal from the
team for the balance of that season.
Game officials and the league will strictly enforce this policy.
